1. Introduction
Organizations already manage risk. They run assessments. They schedule audits. They investigate incidents. Between those events, operations continue.
That interval is where exposure lives.
A register updated last quarter does not describe today’s open conditions. An audit completed last month does not show whether unresolved risk is rising this week. An incident report arrives after something has already failed. Closure rates and open-item counts can look acceptable while weighted unresolved conditions quietly accumulate.
Traditional risk management is necessary. It is not continuous. Leaders still need a current reading of operational exposure while work is underway: not only after the next review cycle, board pack, or investigation.
Why Pulse? Because assessments answer what could go wrong. Audits answer whether controls were in place at a point in time. Incidents answer what already went wrong. None of those, by themselves, answer the daily operational question: what unresolved weight are we carrying right now, and is it getting heavier?
Check the Pulse exists for that gap. The methodology is the core intellectual asset. Software is one implementation of it. This brief defines the method: not a feature catalog.
2. The Gap
Most organizations can answer: What risks have we assessed? Fewer can answer, at any moment: What weighted exposure is open right now: and which way is it moving?

The gap is not ignorance of risk. It is the distance between assessment and monitoring.
Risks are identified, scored, and filed. Controls are designed. Registers grow. Then attention often shifts to the next workshop, the next audit window, or the next incident review. Meanwhile open conditions stay open. Some are closed. New ones appear. Without continuously updated Pulse Exposure, leaders rely on counts, anecdotes, and gut feel. Those matter. They do not give a shared, comparable reading of exposure mass and direction.
When monitoring is episodic, three failures are common:
- Stale picture: the register reflects the last review, not the current state of open work.
- Count without weight: many low-weight items look like “busy”; a few high-score items may dominate real exposure.
- Late signal: deterioration shows up in lagging indicators or incidents after the operational window to act has narrowed.
Pulse names the practice of closing that gap: observe continuously, score consistently, keep an active register, and watch Pulse Exposure as a continuously updated weighted operational exposure measure, not as a static list. The aim is situational awareness for people who must act while the operation is still running.
3. Existing Risk Management
Pulse does not replace ISO 31000, enterprise risk frameworks, or domain systems (SMS, ISM, HSEQ, compliance programs, cybersecurity GRC stacks). Those systems identify risk, set appetite, design controls, and provide assurance. Pulse uses the same basic language they already teach:

Assessments and audits remain the backbone for identification, control design, and assurance. Pulse sits beside them as a monitoring layer. It does not redefine residual risk for the enterprise. It tracks what is still unresolved in the operation and how that exposure changes day to day.
We do not invent a new risk formula to displace yours. We apply the organization’s Likelihood and Severity model to active observations and derive Pulse Exposure from what remains unresolved. If your organization already uses a 1-5 or 1-3 scale for L and S, keep it consistent within a domain. Consistency of scoring discipline matters more than which integer grid you chose.
What Pulse deliberately does not do: replace policy libraries, replace audit programs, or claim that Pulse Exposure alone equals a complete risk management system. It answers a narrower, sharper question: continuous operational exposure: and leaves the rest of the control stack intact.
4. Pulse Methodology
The method is a chain. Each step feeds the next. Skip a link and the reading becomes incomplete.
Observation
Something that carries risk if left unresolved. It is concrete enough to act on: what was observed, where, by whom, and when. Observations are the unit of state change. Without fresh observations, Pulse Exposure is a fiction of last week’s paperwork.
An observation is not a completed investigation report. It is an active condition that may originate from operational conditions, audits, external events, supplier issues, regulatory changes, or other relevant sources: unsafe state, open corrective action, untreated finding, overdue remediation, unresolved control failure, or whatever the domain calls the open risk-bearing item. The method is domain-independent; only the sources and labels change.
Risk Score
Each observation receives a Risk Score using the organization’s defined Likelihood and Severity model. The resulting score represents its contribution while the observation remains unresolved.
Items that no longer require active monitoring contribute zero to current Pulse Exposure. Reopening or revising a score changes contribution immediately. Scoring is judgment under a shared scale: not a claim of precision beyond what the operation can honestly assign.
Active Risk Register
The working set of unresolved observations. Not the full historical archive. Only what still loads the operation. The active register is the substrate of Pulse Exposure. If tracked exposure still requires active monitoring within defined scope, the item belongs here; if not, it does not.
Pulse Exposure
Pulse Exposure is derived from the current state of unresolved observations and their assigned Risk Scores. It represents the organization’s current operational exposure within the defined monitoring scope.
One reading of current operational exposure: not a catalog of every risk the organization has ever assessed. It rises when unresolved weight rises. It falls when unresolved weight is removed or reduced.
Trend
Direction over time: rising, stable, or reducing. Pulse Exposure matters; movement often matters more.
Rising exposure means conditions are being added or persisting faster than they are being resolved or otherwise removed from active monitoring. Reducing means resolution and control are outpacing new unresolved weight. Stable means little net change: which can still hide a bad mix if high-score items replace low-score ones. Trend is the early signal that something in the operational rhythm has shifted.
Dig-in
Where that exposure concentrates, so attention has a target: by severity band (High / Medium / Low), by the active Register, and by category. Section 6 expands this.
History
Observations leave active Pulse Exposure when tracked exposure no longer requires active monitoring within defined scope: resolved, removed, transferred, accepted, or otherwise no longer active. They remain as record. Administrative closure alone does not automatically remove risk from Pulse; the observation leaves only when that monitoring criterion is met. The Pulse reflects state; history preserves accountability, learning, and audit trail. Leaders should not confuse a healthy Pulse with an empty history: or a busy history with a healthy Pulse.
The loop
Unresolved items load the Pulse. When an item no longer requires active monitoring, its weight leaves Pulse Exposure. That is the operational loop:
observe → score → register → Pulse → dig in → close → watch Pulse Exposure move.
Close does not mean the underlying risk has vanished by paperwork alone. It means the observation no longer belongs in the active monitoring set under the scope defined for that Pulse.
The method is domain-agnostic. The discipline is the same wherever you apply it.
5. Why Pulse Exposure Is Different
Pulse Exposure is not total company risk. It is not expected financial loss. It is not a replacement for enterprise risk management (ERM).
It is not every entry in the enterprise risk register. It is not residual risk after all controls on the strategic heat map. It is not a compliance score, a culture survey, or an open-item count dressed up as analysis.
It is current operational exposure: the weighted mass of conditions that still require active monitoring right now.

When an observation enters active monitoring, exposure rises by its Risk Score. When it no longer requires active monitoring, exposure falls by the same amount. When Likelihood or Severity is revised, the contribution updates. No waiting for the next assessment workshop. Pulse Exposure follows the work.
That binding to observation state is what makes continuous monitoring possible. Calendar milestones still matter for assurance. They are the wrong clock for current operational exposure.
A practical consequence: two sites with the same number of open items can have very different Pulse Exposure. Weight, not headcount, is the signal. Another: a department can look “quiet” on closures yet dominate Pulse Exposure if remaining items score high. Pulse Exposure exists to make those differences visible without inventing a parallel risk language.
6. Dig-in: Where Exposure Concentrates
Pulse Exposure can rise for many reasons. Leaders still need to know where.
In the product, dig-in starts from the chart home: Current Pulse, then High / Medium / Low severity bands, the active Register, and category filters. That is the operational path from “exposure is up” to “look here.” Methodologically it is the same idea once called Focus Areas: decompose open weight so attention follows contribution to exposure, not vanity open-item tallies.
High count with low weight is not the same as few items with high scores. Attention follows concentration. If one severity band or category owns most of the current exposure, that is where supervision, resources, and closure pressure belong first. When contributions are even, the signal is balance: not a false single hotspot that forces a theatrical “priority” onto noise.
Dig-in does not replace judgment. It shortens the path from the Pulse reading to the working set that still loads it. Used well, it also prevents the opposite error: treating every open item as equal when Likelihood and Severity scores say they are not.
Categories should stay stable enough to trend. Constantly renaming them destroys comparability. Choose a taxonomy the operation already understands, score honestly, and let concentration: not politics: set the first look.
7. Applications
One method. Several domains. The chain does not change; only what you observe and how you label categories.

Scoring scales and category labels may fit the domain. A safety Likelihood scale need not match a cyber Likelihood scale across products: but within one domain implementation, scoring must be consistent or Pulse Exposure misleads.
Products such as Safety Pulse implement this chain for a specific domain. They are implementations of the method, not a different methodology per brand. The brief stays at method level on purpose: applications prove transferability; they are not a product catalog.
8. Pulse Hub
A future Pulse Hub would sit above domain modules: one place to see Pulse Exposure and movement across Safety, Sustainability, Compliance, Cybersecurity, and further domains as they are added. Same method, shared visibility for leaders who own more than one operational risk surface. That is direction: useful to state so the multi-domain logic is clear: not a claim of a finished multi-domain platform today.
9. Closing
Organizations already know how to assess risk. Assessments, audits, and investigations will remain. They should. The harder daily question is whether operational exposure is being watched while conditions are still open: before they harden into incidents, findings, outages, or regulatory pain.
Records describe what was documented. State describes what is happening. Check the Pulse is built for state.
